Radon Health Risks: What the Evidence Shows

Radon is the second leading cause of lung cancer in the United States. The mechanism is well understood, and the risk scales with concentration and time.

How radon damages lung tissue

Radon gas itself is largely inhaled and exhaled. The damage comes from its decay products — polonium, bismuth, and lead isotopes known as radon progeny. These are solid particles that attach to dust and moisture in the air, lodge in the lining of the lungs, and continue decaying there. Each decay emits an alpha particle, a heavily ionizing form of radiation capable of breaking DNA strands in nearby cells. Enough of that damage over enough years can produce cancer.

The scale of the problem

  • The EPA attributes roughly 21,000 lung cancer deaths per year in the US to radon exposure.
  • Radon is the leading cause of lung cancer among people who have never smoked.
  • It is the second leading cause overall, after smoking.
  • The risk is well documented in both occupational (miner) studies and residential case-control studies.

Why smoking multiplies the risk

Radon and tobacco smoke do not simply add together — they act synergistically. Smoke damages the same tissue and impairs the lung's ability to clear particles, so radon progeny sit longer against already-compromised cells. At the same radon concentration, a smoker's lifetime lung cancer risk is many times that of a never-smoker. Households where anyone smokes have the strongest case for mitigating even at levels between 2.0 and 4.0 pCi/L.

Exposure is cumulative. Twenty years at 4.0 pCi/L is a materially different exposure than two years at the same level, which is why the lowest lived-in level of a home — where people sleep or spend evenings — matters most in testing and in decision-making.

Who is most at risk

  • Anyone who smokes or has smoked, or lives with a smoker.
  • People who spend many hours per day on the lowest level — home offices, basement bedrooms, finished lower levels.
  • Children, who breathe more air relative to body size and have more remaining years of exposure.
  • Households that have lived in the same home for decades without ever testing.

The good news

Unlike most environmental carcinogens, radon is measurable for a small cost and reducible with a one-day mechanical fix that has a verifiable result. There are very few health risks where the path from unknown to resolved is this short.
